You can access & answer a form in two ways:
Login on the client portal
Email Notification
Client Portal
Login on your client portal to access the shared form.
Click the Forms Tab seen on the left navigation bar.
To view and answer a form, choose and select a Form in the Pending tab.
The form will open. You may now answer the questions about the Form.
Click the Submit button.
Note: You can also click the Submit button below the Form modal.
To view submitted forms, click the History Tab found in the Forms page.
You will receive an email notification if your practitioner shared a form in the client portal. Click the Complete Form button. Here’s a sample email:
