The different field types found in the properties section have various use depending on the type of basic field type you’re using. It also depends on how your question is created in order to match what field type suits your Form questions.


Note: Photos shown in the example scenarios on each step are based on the Form visible to your client’s view.


  1. Choose and Select a draft or published form from the Forms tab list.

 

  1. Select a Question to be edited on the Form.

 

  1. If a client needs to respond in a descriptive format, choose:

  1. Single field type - the blank field provided will only be a one-line format.


  1. Multi-field type - the blank field provided will have a paragraph-style 

format.


  1. If a client needs to answer a multiple-choice question, choose:

  1. Single field type - if you allow only one (1) answer among the options provided.


  1. Multi-field type - if you allow multiple answer(s) among the options provided.


  1. If a client needs to pick a date, choose:

  1. Single field type - if you allow only one (1) date to be chosen on the calendar


  1. Range field type - if you allow a range or duration of the therapy session(s) to be chosen on the calendar.


  1. If a client is asked to rate a question, choose:

  1. Scale field type - if you’d like a broken line format. 


  1. Range field type - if you’d like a solid line format. 


  1. To save all changes made, select any of the Save options on the modal.
