Evaluation Forms allow practitioners to complete assessments and evaluations for their clients directly. While the creation process is the same as creating a Basic Form or an Intake Form, Evaluation Forms are specifically designed for structured data collection and analysis and are not intended for client self-intake.
When to Use an Evaluation Form
Use an Evaluation Form when you need to:
- Complete assessments or evaluations for a client.
- Record structured clinical findings and observations.
- Collect data for progress tracking and reporting.
Note: Evaluation Forms are completed by practitioners and are not intended to be filled out by clients through the Client Portal.
How to Create an Evaluation Form
The process for creating an Evaluation Form is the same as creating any other form in the Form Builder.
