What are TB Forms?
Introducing TB Forms, the advanced form builder by the Therabyte team. With TB Forms, you can easily create diverse forms to collect information from your clients. Not only that, but you can also share specific sections of your forms with your clinic or other Therabyte users.
What Can Forms Do?
- You can create online intake and consent forms that can be shared with new clients, via the landing page, or accessed via a button on your existing landing page (website).
- You can share other client related forms with existing clients via email or via the client portal.
- Therabyte forms use a combination of Basic Fields and Built-in Blocks, allowing you to create custom forms with exactly the fields you need.
- Built-in blocks simplify data entry by automatically mapping information into the client’s chart, helping you maintain complete and accurate records with minimal effort.
- Therabyte forms support e-signatures but do not allow uploading a PDF form for digital signing. If you need to share a PDF with a client, you can send it via a note, and the client can upload the completed document back to you through the client portal. See instructions here.
The Three Types of Forms
- Self-Intake Form
This form is available directly through your clinic website and is designed for new clients to fill out before their first appointment. It helps collect essential information ahead of time. You can also customize self-intake forms for existing clients—allowing you to create multiple versions to include clinic policies, consents, or other important agreements. - Basic Form
The Basic Form is ideal for existing clients. It’s simple to share and complete through the client portal. Unlike the self-intake form, it doesn’t require a built-in client profile block, as the system automatically links the form to the specific client it’s shared with. Use this form for surveys, feedback, or any general questionnaires. - Evaluation Form
The Evaluation Form functions similarly to the Basic Form but is intended specifically for assessments and evaluations.
Anatomy of a Form
Therabyte Forms are designed with flexibility in mind. Each form is made up of multiple Steps or Pages, helping you organize information into sections.
Within each step, you can add Blocks, which group related fields together. Inside each block, you can include various types of input fields—such as text boxes, multiple-choice questions, dropdown menus, or scales/ranges.
To simplify setup, Therabyte also offers Built-in Blocks with pre-set field structures. For example, the Client Profile Block helps ensure consistent and accurate collection of client information, streamlining your workflow.

The Form Editor
The Form Editor is the main workspace where you create and organize the layout of your form. It allows you to:
Drag and drop field types to design your form.
Edit the content of each field, aside from its properties.
Preview the form using either the Preview All button or the Form Preview button.
Step (Form Page)
Each Step represents a page of your form. You can customize the Step Title to suit your form’s structure.
+ New Step
Use this to add additional pages to your form. This helps you organize long forms into multiple sections based on headers or topics.
Preview All
Click Preview All to view a full preview of all the basic fields and built-in blocks you’ve added to your form.
( + ) and ( x ) Buttons
These buttons allow you to:
Add (+) new basic field types within a row.
Delete (x) field types from a row.
You can add multiple fields in a single row if needed.
Block
A Block is a subsection within a page. You can:
Rename the block or leave it untitled, depending on your preference.
Organize your form into sections using blocks.
Block - Preview
Click Preview to see how all the fields within the block will appear to clients.
Block - 3 Vertical Dots (More Options)
This menu allows you to:
Duplicate, Delete, Move, or View Properties of a block.
Save as Custom Block for repetitive sections you frequently use in other forms.
+ Add Block
Use this to add a new section to your form. You can choose:
A generic block, or
One of the pre-built block types from the dropdown.
Signature
Toggle this ON or OFF to include a signature field at the end of a block. You can also enable this in the Properties section.
+ Row
Click + Row to select the type of basic field you’d like to add to a specific row. This is an alternative to dragging and dropping from the sidebar.
Row - 3 Vertical Dots
The options here are specific to the selected row. You can:
Duplicate the row
Toggle Preview
Delete the field type from the row
