How to create a container for FAQ and Policies

Create a repository for a clinic’s FAQ and Policies

  1. Click Management > Forms in the left-most menu.

  1. On the Forms Page, click the New Form button.

  1. Click the Basic Form box. This is used when creating a general-purpose form we will need for our container, and then click the Create button.

  1. Click the Let’s Do It button.


  1. A blank form will be displayed, ready for you to start editing.

  2. You may change the name of the form. Please enter the title 'FAQ and Policies' in the upper left section..

  1. Drag and drop the Text/Description object to the Form Editor.

  1. Type the FAQs on the Block textbox.

  2. Type in the FAQ Question and Answer to the Text/Description. It is better if one question and answer per textbox.

  3. Click the Ellipsis button and click Duplicate from the dropdown list

  1. Rename the Block header as ‘Policies’. Rename the questions with the organization’s policy.

  1. You may click the "Form Preview" button to see the preview of the form when it is live on the web.

  1. Click the Close Preview button to return to design mode.

  1. Once you have completed the FAQ and Policies list, you may now click the Save button, then from the option list, click Save and Publish.

  1. Click the Live toggle to make the form visible on the landing page.

  1. Since this is a new Form Category, you simply need to click the Add (+) button at the bottom of the dropdown list. It's an easy step that allows you to include the new category.

  1. Type in the title of the category, provide a brief description below and then click the Save button. 

  1. Click the Confirm button to proceed.

  1. Click the Continue button.

  1. To set this version of the form as the active one (in case there are multiple versions already existing), simply click the Active Version Toggle button. 

  1. Click the Continue button.



  2. Click the Settings button.


  3. Choose from the Category list and click the Confirm button.


To View or access the FAQ and Policies form.

  1. Access the landing page of the Clinic’s Website.

  1. Click the FAQ and Policies from the Category list.

  1. Click the FAQ and Policies from the Form list.

  1. Now you'll be able to see the FAQ and Policies form displayed. User can click the X button to close this form.
